Community Grants
Grants for breast health education, breast cancer screening, diagnosis and treatment support programs are provided by Komen Affiliates with funds raised through events such as the Susan G. Komen Race for the Cure.

The Hawaii Affiliate awards grants only to nonprofit organizations, educational institutions and government entities in Hawaii. In order to ensure the funds raised make the greatest impact, Komen

                                                                                                Affiliates work with local medical experts and community leaders to conduct community needs assessments. These assessments are then used to establish local grant application and review processes consistent with our Affiliate’s service area needs and the Komen vision.

The Hawaii Affiliate of Susan G. Komen for the Cure has taken an active leadership role in funding grants to benefit area families. Programs funded from affiliate fund raising events range from educating women about healthy behaviors and early detection to providing treatment services to women and men who are low income/uninsured. The affiliate empowers women to take charge of their own health through participation in breast health seminars and outreach support targeting underserved and hard to reach populations.

25% of the proceeds from affiliate fundraising events in Hawaii fund research programs to find a cure for breast cancer. 75% of the proceeds from affiliate fundraising events are used to fund mammograms, breast cancer treatment, and education in Hawaii.

The Susan G. Komen For the Cure - Hawaii Affiliate is pleased to announce the 2007-2008 Komen Hawaii Grant recipients. Komen grants are awarded annually for programs that focus on breast cancer education, prevention, awareness and treatment. In the Affiliate’s 10+ year history, over 50 of Hawaii’s community-based health centers, hospitals and programs have received funding from Komen to help fulfill the mission of eradicating breast cancer as a life-threatening disease. In 2006, Komen Hawaii met its $1M mark in the funding of Hawaii programs.

The Hawaii Affiliate works to expand Komen’s Promise throughout the Hawaiian Islands by supporting and promoting collaboration among Hawaii’s healthcare providers and organizations; enabling implementation of culturally sensitive education, screening and treatment programs; and obtaining and providing resources for the activities that will make our vision a reality.

GRANT PROPOSALS—2008 – 2009

AMERICAN CANCER SOCIETY
Friend to Friend Keychain Project—ACS has requested this money for the purpose of buying the supplies needed to produce 15,000 key chains by volunteers for distribution to local health care organizations for the purpose of promoting dialogue regarding breast care health and the importance of early breast cancer detection. The culturally appropriate ipu key chains are made up of various size beads representing the different size lumps found through breast cancer screening methods.

BAY CLINIC, INC.
New Breast Cancer Screening Clinic for Medically Underserved Women—This project will initiate the first specialized women’s health clinic in rural East and South Hawaiian Island especially targeted toward at-risk women of Native Hawaiian/Asian American/and Other Pacific Island descent. The purpose of the program is to decrease barriers to care and increase the number of women receiving breast cancer screenings.

HAMAKUA HEALTH CENTER
The Breast Health Project—This project is intended primarily to fund mammograms and follow-up diagnostics procedures for 75 uninsurd/underinsured women of North Hawaii.

HAWAII MEDICAL CENTER FOUNDATION
Breast Cancer Screening for Medically Underserved Women ages 40 – 49—This project will provide free mammograms and clinical breast exams for asymptomatic women ages 40 – 49 at two sites: Hawaii Medical Center East (Nuuanu) and West (Ewa Beach).

KOKUA KALIHI VALLEY
Breast Cancer Prevention for the Women of Kalihi Valley—This community-based program will include outreach to women where they live, work, and worship. Activities include: 1) Breast health awareness for 800 through participation in community health fairs and events; 2) Breast health education provided to at least 120 women through churches, community organizations, Kokua Kalihi Valley main clinic, and homes; 3) Peer-to-peer programs will encourage at least 25 women to recruit friends and family members to received breast cancer screenings; and 4) mammography services for 50 women.

KAPIOLANI BREAST CENTER
Prevention of Breast Cancer in Hawaii’s High Risk Women—This grant will help fund the only integrated program in Hawaii to offer cancer risk reduction to high risk women, aimed at preventing breast cancer. It will provide access to this program those women who lack medical insurance, giving special emphasis to better integrating this prevention program into native Hawaiian and Pacific Islander communities.

LANA’I COMMUNITY HEALTH CENTER
Breast Health, Education, and Screening—Funding this project will provide free clinical breast exams, breast health education, and educational materials to meet the needs of the women of Lana’I, with a focus on the Filipino, Native Hawaiian, and other Pacific Island populations. Because there is no mammography on the island, referrals are made, and patients are transported to either Maui or Oahu, for services.

MOLOKAI GENERAL HOSPITAL
Molokai General Hospital Breast Cancer Enhancement—The purpose of this grant is to improve the continuity of breast care on Molokai and decrease the time interval from diagnosis to treatment by overcoming factors such as geography isolation, insufficient health care resources, economics, limited cancer knowledge, and motivation. A patient navigation system will assist Native Hawaiian women of Molokai (62% of the population of Molokai is Native Hawaiian or part Native Hawaiian) to get culturally appropriate assistance.

WAIKIKI HEALTH CENTER
Breast Health Services Targeting Medically Underserved Women—This funding is to provide breast health services to the following underserved populations: Homeless, Native Hawaiians, Filipinos, Samoans, Tongans, and Micronesians. Staff reflects ethnicities and cultural backgrounds of populations served, and the program combines Western medicine and traditional Hawaiian healing practices.

WILCOX MEMORIAL HOSPITAL
Women’s Health Program—This program will provide breast cancer screening to uninsured women in need of a mammogram or breast ultrasound test. An outreach program will enroll 40 uninsured women age 35 and older to help them access early breast cancer screening.



Outreach Program

This program facilitates free information and education focused on the importance of breast self-examination (BSE) and mammography. The program helps women understand why these screening methods are so important and dispels myths.

GRANT INFORMATION

Small Grants may be awarded to nonprofit organizations, educational institutions, and/or government agencies.

Requests should comply with the following criteria:
* Specific to breast health/breast cancer
* Relevent to the mission of the Komen Foundation (The mission of Susan G. Komen for the Cure is to eradicate breast cancer as a life-threatening disease hy advancing research, education, screening and treatment.)
* Funding request is less than $2,000
* Willingness to acknowledge support of Komen Affiliate
* Report financials and project progress to Affiliate
* Allow participation of Affiliate representatives where appropriate
* Not for a fund raising activity
